Unable to get output in attached tex file

There are problems with headers and footers. You can do this with internal exam class commands. Please consult the documentation, section 2.6 (Headers and footers)
The smart way: Calm down and take a deep breath, read posts and provided links attentively, try to understand and ask if necessary.

Please get used to giving an adequate problem description. That means that you should echo error messages from the log file (*.log) or mention any other irregularities. Although you have given an example, it can be a local problem related to your TeX system that others can't reproduce.
