Document Classes ⇒ Going crazy with thesis typesetting - TOC, LOF, LOT

Hi Guys,

Sorry for the frustrated tone of the post. I am going crazy with all the requirements of graduate college. I am using a report class. How do I include the words 'CHAPTER' and 'Page' at the beginning of every page in TOC? Similarly, for LOF they want 'Figure' and 'Page' and for LOT they need 'Table' and 'Page'. Please help me.
