I am trying to add a box over an image to serve as annotations with no success.

Anyone knows how to?

Without minimal working example that shows, what you've already tried, I can only give two tiny suggestions:

• Use package overpic to place elements like a \framebox over the image.
• Use TikZ to print the image and draw onto that image.

BTW: There are already questions like this, e.g. write text into a graphic
