Page Layout ⇒ grid.tex QUICK LINKS not showing

I downloaded the grid typesetting package from CTAN, http://www.ctan.org/tex-archive/macros/ ... ntrib/grid and I am having trouble generating the quick links in the top right of the header section.

To create a MWE, the following files need to be downloaded from the CTAN archive: grid.sty, grid.text, rvdtx.sty and manifest.tex. A makefile is also provided for GNU/Linux users, but I did not use it.

The screenshot of the pdf fle in the CTAN archive shows how the quick links should appear, with a summary of the section headings, compared to another screen shot showing how grid.pdf appears in CTAN. I am using Texstudio 2.6.2 on a Mac, OSX 10.6.8. I tried to compile, build and index multiple times. Everything generates properly except the quick links section.
