Questionnaires with selectable items

Dear Community,

I have created a paper and pencil questionnaire with help of the paperandpencil.sty from QDDS, which is available at http://www.qdds.de/index.php?id=129.

This just worked fine and I experienced no problems. But I want to go a bit further and want to make the created squares selectable inside the pdf-Document, so that someone can fill out the questionnaire on his/her computer and save the results.

I did this with a Adobe Acrobat Pro, but it was a a lot of work, because I had to create every square single-handedly.

So my question is: Is there a package with which it is possible to create multiple selectable items for a questionnaire in a document compiled with LaTeX?

You might want to check out the hyperref manual, section 6 is all about PDF forms with LaTeX.
