I cannot manage to create my footer and header. They are very simple, and they should be and all the pages of my report.
Please find my report on the attached file
If someone can help, it will be very nice!

Which document class are you using? And which package for header and footer, if any? (fancyhdr, scrpage2, titlesec, ...)

And what did you try, where did you have problems?
