Page Num: ! pdfTeX warning (ext4): destination with the same

hello.

I am working from the template http://www-h.eng.cam.ac.uk/help/tpl/tex ... PSnPDF.zip

I am receiving the error:

Code: Select all

! pdfTeX warning (ext4): destination with the same
identifier (name{page.1}) has been already used,
duplicate ignored
when I type more than 1 pages worth of text in a chapter.

My problem is I cant see how the suggested solutions can help me. My PDF document is large (lets say 500+ pages). In the zip file of the min working example I can see that plainpages=false into the hyperref options as descirbed on the wiki.

I am confused by the maktitle suggestion, as I dont have that anywhere. The other suggestion they use is to suggest the page counter is reset every page. Do they mean doing this manually as surely this cant be done for a large ducument?
