mdframed - page break

Hi,

I am referring to the code on this page: http://tex.stackexchange.com/questions/ ... 215#318215

Is there a possibility, that the heading after the pagebreaks is different, e.g.
First page: Concept 5: Algebraic Identities
Next page: Concept 5: Algebraic Identities - Continuation?
