In LyX, go to Document > Settings > Modules and add the module "Custom Header/Footerlines". Pay attention to the note that is listed there:

Follow those instructions. Then in the upper-left combo box (it will currently say "Standard" if you are in a blank new document), choose one of the headers.Adds environments to define header and footer lines. NOTE: To use this module you must set the 'Headings style' in the menu Document Settings -> Page Layout to 'fancy'!
Package(s) required: fancyhdr.sty.
