including style.ist for customized index
I'd like to customize my index a little bit.
The final index should have 2 colums per page and look like this:
A
abc.....5,6
aef......7,8
asdfe.....4
Now up to now I know that I need to define these custom style in a .ist-file, which i have to include via -s file.ist while using makeindex.
Can you tell me, how I can include such a file in Lyx? When adding the information to the preamble an error occurs telling me that no document begin has been found. When adding it to the main text, the error "only usable in preamble" pops up.
The third option I tried was adding it to the document options -> index. There I can select "makeindex" by dropdown menu and then I'm able to insert some options. But when adding "-s file.ist" or "makeindex -s file.ist" or "\makeindex -s file.ist" nothing happens. When taking a look into the plain exported .tex file, I can't find my option - so that's why noting happens.
So, how am I able to customize my index? If I need a special style, how can I include the file?
I am using LyX and MacTeX on a Mac computer.

I have to say that I don't have any solution after hours of searching the web to include my .ist style-file into LyX makeindex.
I think it'll work something like that, but this here won't work (my index just disappears after compiling):
