Page Layout ⇒ book layout: headers left, text indented, separation lines

I am looking for a template that could generated a book-style document similar to this: http://users.uj.edu.pl/~ufdzierz/PracFot/SHG_Dood.pdf

with:
- header aligned left
- text indented
- header separation line over the whole page
- footer separation line over the whole page

Searching in this forum and google did not help so far because I didn't know what exactly to search for...

Several possibilities in form of classes and packages.

• The fancyhdr package
• The titleps package
• The classes from KOMA-Script (with »scrpage2«)
• The memoir class (built-in mechanisms)

A very good starting point are the respective manuals.
