MakeIndex, Nomenclature, Glossaries and Acronyms ⇒ apacitex | No Author Index

Dear all,

I am using the apacite (using it as apacitex) package which should (almost) automatically generate an Author Index. I have tried several things, but cannot get it to work. The *.adx file is created and looks okay. However, no *.and file is produced and thus I do not get an author index in my document. The subject index is working fine.
This is the sequence I am following: "First, LATEX, which puts citation entries in the .aux file. Second, BibTEX, which generates the bibliography in the .bbl file. Third, LATEX (at least) twice, which puts index entries in a .adx file, Fourth, MakeIndex , which uses the .adx file as input and puts the formatted index in a .and file, which can be processed by LATEX. "

I am not very experienced, so apologise if I'm forgetting something really stupid. I have attached a minimal working example and the log file. I hope it contains everything.

Why doesn't LaTeX produce the .and file / the Author Index?

Hmm... Haven't had a reaction on this topic yet and have also not yet solved it myself. I noticed that the MWE was gone, so here it is again. Hopefully someone else can help me.
