General ⇒ LoF, LoT, Acknowledgments and others in ToC

hello,

i have a question.

What's the policy with the following items:

• Acknowledgment
• Abstract
• Sommario (Italian abstract)
• Table of Contents
• List of Figures
• List of Tables
• List of Acronyms
• List of Symbols
• Chapters
• Bibliography
• Appendices

Is this the standard order of the items in the thesis?
Which of those should have an entry in the ToC?
So far of lists and the table of contents itself do not appear in the Table of Contents. Do you think this is the usual way?
