LyX ⇒ Blank Chapter Cross-references

This used to work, but now my chapter Cross-references are blank.

Not trying anything fancy, just a simple: "also see Chapter 3, on page 35"

The page numbers are correct, but the Chapter numbers are missing;
"also see Chapter , on page 35"


Any idea what's going on here???

OK revision in thought:

It seems that the cross-reference issue is merely a symptom of a larger problem: Chapter Numbers have vanished entirely in this document. They correctly show in LyX, but when printing or making a pdf, there are no chapter numbers. All sections are 0.x, regardless of what chapter # they live in. section 4.3 is printed as 0.3 etc.

Any suggestions?
