I was expecting that you might notice something in the actual code that might be causing the problem. Posting a mwe (as I previously did) is often helpful but sometimes we need to check the actual code used in order to point out any inconsistencies.
I'm glad we sorted the problem out. But in response to this point, if you read the link I posted to, it describes two ways of creating minimal working examples. One way is from the ground up. You did that, but it didn't generate the problem. The other way is from the top down; beginning with the full document and removing pieces from it, bit by bit, until the problem disappears. If you had tried that, you would easily have solved this problem for yourself.
