Index layout

Dear (La)TeX friends.

First of all, please take a look at the attached screen dump (well, part of one).

I would very much like to generate an index like the one in the picture. The first column is the English phrase (the one used in the notes), the middle column is a Danish translation, and the last column is the paragraph number.

Can anyone help?

PS. I am a totally n00b when it comes to generating indices.

I have now figured out how to create an index, but I still have problems, though: I have no idea what to put in the .ist file in order to make the generated index look like the one in the screen dump.

Any help is much appreciated.

I have dropped the idea since I cannot get it to work.

Another problem: Can anyone tell me what to change in Juanjo's (or maybe swiftarrow's) code at this thread in order to write an index entry in italic mode (I would like the corresponding page number på to be in upright text mode) by default?
