Help on typesetting a header

I am curious how to achieve the vertical textblock for chapters. I took this from a pdf generated by latex. Textblock for every new chapter is lower (precisely the hight of the block lower). Is right on odd pages and left on even pages.

Any easy way to do this or must it be done by hand?

Take a look at the thumb and thumby packages. There's also a similar example discussed in the fancyhdr package documentation, page 20ff.
