I apologize for the somewhat awkward title, but my questions are related to a thesis, and since you have more experience than I do, I would very much like to hear your opinions on the matter. So keep in mind that the following questions are regarded a thesis:
1) Would you prefer small or normal-sized equation-numbers?
2) Would you prefer equations only to have a corresponding equation-number if it is referenced, or should all equations have a number?
3) What about a header? I've been thinking for a long time whether I should include one or not, but I can't find any inspiration. If I include one, it has to be very minimalistic.
Any input is greatly appreciated.

I can't say that any of the below comes from experience... just personal preference.
1. Normal sized numbers.
2. If there's any chance your committee or any other readers might want to discuss the equations you yourself don't refer back to, it still helps if they're numbered. If it's not something they might want to discuss, then you can skip them.
3. If your thesis has chapters you could put the chapter names up there, but a header is not necessary. I wouldn't have both a header and footer, so if you were to use a header, I'd put the page numbers up there too.
But you might want to check with your University's policies on such things. There's a good chance that you don't have a choice on what the headers look like. (Certainly you didn't at the school were I submitted my thesis.)
