i've the same problem.
I'm using with lyx the elsevier class document, but i need vancouver style (in the reference section: author, date, etc...).
I create with makebst a file vanc2.bst (in attachment).
If i use this style i can compile the file, but i reveive the following error: (author?) [1 ...
