I am using the apacite (using it as apacitex) package which should (almost) automatically generate an Author Index . I have tried several things, but cannot get it to work. The *.adx file is created and looks okay. However, no *.and file is produced and thus I do not get an author index ...

It's interesting, because I had previously specified the table width, but got an error as well. Anyway, it works indeed, so that's great!
