General ⇒ creative commons thesis template?

Dear LaTeX users,

I am looking for a (PhD thesis) template that would contain creative commons licensing terms. Any tips? Other templates with CC-notes (text & logos) go as well.

I would like to check some examples and potentially reuse the code.

Here is an example of a CC-licensed thesis, but this one was not written in LaTeX (personal communication):
