May I make the humble suggestion of asking the author(s) of those slides? There are three lecturers listed for the course those lecture notes came from, so I would expect a maximum of three e-mails before you got one of the following answers: (1) "We made that ourselves, and it is not intended for use by the general public," (2) It is a modification of the 'SNAFU' theme with xxx changed to XXX and yyy changed to YYY," (3) "It is the 'FUBAR' theme, available from [Web site]."
