I would like to have a sample latex document that I can download to use as a guide for preparing a handout for my students on a certain topic in math. I do not want it to be in article nor book form. And please do not respond by saying that there are lots of such documents on the internet without stating at least one particular precise reference.

What have you tried so far? Or do you expect a template written from scratch? What about you other question [1]? The web is full of templates.
