There is probably a very easy solution for this, but I can't find it. Both in my dissertation project and in a completely virgin/vanilla document using all of the default LyX settings, the index list fails to appear. Please see the screen shot. The preamble is empty and everything else is standard. No page beyond the two pages shown is generated. In my dissertation project, the page that should contain the (start of the) index list is simply skipped. Any help is appreciated!
