I had a similar problem as the OP. Mainly because I rarely use MiKTeX and installed it a while ago.
localghost's reply helped point me in the right direction. Unfortunately all start menu entries were removed somehow, but after half an hour of searching I discovered:

Thanks, Local Layout seems to do what I want! Is there a way to persist over multiple documents? Right now it seems to reset for every new document I make. I would like to apply
To every new and old file I open.
