Did you read TeX on El Capitan and the PDF with instructions there? Did you correct the settings for the path, and how?

I only can see that LyX doesn't find your TeX installation (the document classes) but not what you already tried.

This is the error I get when opening the LyX 2.1.4 and open a new file.

I've installed LyX 2.0.3 and the problem still persists. On both the most recent version of LyX and the two versions of LyX that I've installed, this issue occurs. Screen shots of the issues are attached.

Worked perfectly. Specifically doing this (which was mentioned in the link):
"if one goes in
Settings\File Handling\File Formats and selects “PDF (pdflatex)” in the second
to last dropdown menu in the settings, the one saying “With TeX fonts”, that
makes the icons “live again”, at least it did ...

\maketitle

this puts the TOC into the argument of \date .
Move \tableofcontents and the \pagebreak commands after \maketitle .
