I had this problem also.

The rest of the TOC was displayed when I included the lincsdoc.sty as follows:

\usepackage{llncsdoc} % allows for a toc!

However, the title and author still appear as the first item in the TOC.

I have renamed the title that appears in the TOC with the command ...
